Vaccines
For individuals aged 65 years and older to be considered up to date on their COVID vaccines they must have:
- Received 2 doses of the ‘25-‘26 COVID vaccine
OR
- Received 1 dose of the ‘25-‘26 COVID vaccines within the last 6 months
Just a reminder, vaccines will only be delivered on the first delivery, Monday-Friday!!
GLP-1 Bridge Program

For weight loss only: Provider must include an obesity diagnosis code and indicate “SEND TO BRIDGE FOR WEIGHT MANAGEMENT”

Diagnosis that are not eligible for bridge program as they are covered under Part D plan: Type 2 DM, obstructive sleep apnea, metabolic dysfunction associated steatohepatitis

Medications covered in program: Foundayo® (orforglipron), Wegovy® (Semaglutide injection and tablets), and Zepbound® (Tirzepatide KwikPen®).
LTC/SNF Survey Trends
- AIMS assessment needs to be done on admissions and we recommend quarterly thereafter.
- Drugs that need AIMS: Antipsychotics, Reglan (Metoclopramide), and Compazine (Prochlorperazine) even for alternative diagnosis
- Please ensure psych consents are completed for psychotropic medications continued upon admission, new orders, and dosage increases
- When passing meds, ensure what the pharmacy dispenses matches what is on the EMAR. If not, please clarify with the pharmacy or nursing leadership.
- F605 tag: Rationales for non-CMS approved diagnosis on antipsychotics (approved diagnosis include Schizophrenia, Huntington’s, Tourette’s) must contain more information than “benefits outweigh risk.” The rationale needs to include details such as: behaviors targeted, non-pharm interventions in place, do they present harm to themselves or others, previous therapies that have failed, etc.
- F605 tag: PRN psychotropics must have a stop date (there are no exclusions for hospice or any other diagnosis)
- Antipsychotics are limited to 14 days, for it to be continued the resident must be reevaluated by the provider every 14 days.
- Other PRN psychotropics can be extended beyond 14 days if they have a specified date and rationale.
- If a resident has an order to self-administer medications, please ensure that the proper assessment is completed.
- Insulin pens, inhalers, nebulizers, etc. must be labeled with date open and expiration/discard date. Refer to Short-Date Sheet
