What is long term care pharmacy and what are consultant pharmacists?
The first part of understanding who we serve is to understand what we do. US House Bill H.R. 5632 defines it the following way: “The term ‘long-term care pharmacy’ means a pharmacy licensed under applicable State law that is able to provide enhanced pharmacy and clinical services to individuals who require enhanced medication services and reside in a facility.” AuBurn Pharmacy Long Term Care partners directly with a variety of types of these facilities and homes, to provide those critical enhanced pharmacy services that cater to each of their unique needs.
We can do this by employing a staff of highly-skilled consulting pharmacists who work closely with the nursing staff and directors of the long-term care facilities with a focus on improving patient outcomes through the judicious use of medications. These consultant pharmacists are committed to providing person-centered care, taking into account the complex interrelationships between disease states, nutrition, medications, and other variables. They are essential clinicians on the health care team, and influential decision-makers in all aspects of medication therapy.
